It reviews the patient case study after the first visit. The diagnosis is done on the basis of the case study.
Follow-up
  The further investigations of the case study are done. The response of the medicines or therapy prescribed in the prior visits is reviewed.
 
History-emergency
 
•  Chief complaints These are the complaints in terms of symptoms either put forth by the patient or the patient's relatives. These complaints are further analyzed by the doctor for diagnosis
•  History of present illness Investigation of the current lifestyle of the patient, whether the patient is alcoholic or a smoker or on some medication, his behavior within the family, his office environment etc
•  History of past illness It refers to the investigation of whether the patient has undergone through any stressful situation like accident, surgery, loss of family member, hectic job schedule etc  
•  Family history It looks into whether the family has any heredity of having similar illness
•  Personal history It looks into the lifestyle of the patient, his family or work environment or whether the patient had been suffering from some illness
•  Social history Investigations in relation to the social behavior of the patient
•  History of alcohol Investigation regarding alcohol consumption by the patient, which also includes the frequency of alcohol consumption
•  Physical exam The height, weight, etc of the patient are recorded for further analysis
•  Mental status exam  

This consists of IQ tests to check the mental status of the patient

•  Order investigation

This gives the order of the tests to be done of the patient

•  Lab report entry

The tests conducted of the patient are compiled and kept in the records for reference.

•  Diagnosis Based on the lab reports the diagnosis is made which further decides what treatment is to be given to the patient
•  Therapeutic plan

Based on the diagnosis and the tests conducted of the patient, a therapeutic plan consisting of the medication or therapies like shock treatment are done

•  Electro slip Electro slip is a kind of psycho therapy or a part of treatment in which the doctor conducts a test through m/c and notes all results for further investigation
